Managing Vulnerable Frequent Service Users

We are working with the Academic Health Science Network North East and North Cumbria and the Strategic Clinical Networks to spread learning and best practice amongst crisis concordat areas from a project initially delivered in partnership with Tees Crisis Care Concordat.

The Tees project focused primarily on identifying and analysing vulnerable people who are frequent service users, and became known locally as the Cohort 30 work stream as each organisation worked with their 30 most frequent users of services. The project also investigated the possibility of non-emergency transport for people in mental health crisis.

We have now produced a set of guidance protocols and process to enable concordat groups to bring together key personnel and data sources in order to develop system improvements for each identified group of frequent users of service, through facilitated accelerated learning events.
